How complete are first-submitted escalation packets?

A bounded study of missing evidence, owner questions, and correction cycles.

Operations teammates reviewing documented evidence together

Research question: Which required fields are missing when an exception first reaches its authorized owner?

Scope and method: Freeze one template and observe eight consecutive weeks of first submissions, retaining originals, later questions, corrections, queue, shift, risk class, and outcome.

Unit of analysis: one packet submitted for one bounded owner decision. Stable identifiers prevent replies, edits, and transfers from inflating the denominator.

Classification: complete, missing source, missing identifier, missing chronology, unclear question, wrong owner, conflicting evidence, or unusable. Code observable states separately from causal or personal judgments.

Measures: class counts and denominators, question rate, time to usable packet, correction count, and unresolved age. Publish counts, denominators, distributions, and missing observations.

Validation: A second reviewer independently codes a stratified sample and applies the reconciled codebook to unseen packets.

A Philippines-based specialist may collect permitted records and apply the frozen codebook. Internal owners retain privacy, security, legal, financial, employment, policy, and customer-remedy decisions.

Use named access, minimum necessary data, and de-identified reporting where practical. Record exclusions before outcome review and retain correction history.

Limitations: side-channel questions may be absent, queues differ in complexity, and eight weeks may miss seasonal conditions.

Conclusion: Use results to repair fields and routing, not to infer individual competence or causation. Repeat after material changes to tools, policy, routing, or case mix.
